Students at Eastwood Middle School have opportunities to participate in school-sponsored athletics throughout the year. Explore the different options below for more information!

ALL SPORTS REQUIRE 100% COMPLETION IN DRAGONFLY PRIOR TO TRYOUTS

All sports require 100% completion in DragonFly prior to tryouts. This includes a current physical, parent/guardian signatures, and all TCS athletic forms. DragonFly MAX

• Preparticipation Physical Evaluation Form
• HIPAA Form

Medical Information

Parents, if your child has a medical diagnosis such as asthma, seizures, etc., please notify the school nurse of your child's intent to try out for any sport so appropriate medical care can be coordinated during said tryout. The Eastwood nurse line is 205-759-3613.
